CareOregon’s Connect to Care team lives up to its name. Our team helps connect the community to the health care system and answers member questions about CareOregon benefits. When you go to a Connect to Care event in the Portland area, we’ll either answer your questions on the spot – in your preferred language – or connect you to someone who can.
Here are some of the health and social services the Connect to Care team and our partners can help you with:
Answer questions about Medicaid and check your Oregon Health Plan (OHP) status
Refer you to assisters who can help you enroll in OHP

Coordinate your medical, dental and mental health care and appointments
Update your address, phone number and other info
Order a replacement Member ID card
Connect you with trained staff who can offer special help
