Chronic conditions are illnesses that go on for a long time and often don’t go away completely. Living Well with Chronic Conditions is a six-week workshop that teaches you practical skills to manage a chronic condition. Participants share their successes and get to know other participants. Everyone supports each other. The workshop builds confidence around managing your health, staying active and enjoying life. Most workshops are offered free. You can get reimbursed by CareOregon for any workshop expenses. Learn more below.

What will I learn in the workshop?

  • Working with your health care team
  • Setting weekly goals
  • Effective problem solving
  • Better communication
  • How to relax
  • Handling hard emotions
  • Tips for eating well
  • Safe, easy exercise

Workshop description

Length: A six-week workshop that meets 2½ hours each week.

Who should attend: Anyone living with chronic health conditions, including diabetes, arthritis, asthma and heart disease. Family members and friends are welcome.

How will the workshop help? You’ll learn skills to manage your chronic condition, giving you a greater sense of control over your life.

Who leads the workshop? Two trained leaders. One or both of the leaders have a chronic condition.

Locations: Find a workshop in your area. (Follow this link, click on Find a Workshop, and select “Chronic Disease Self-Management Program” from the program drop down menu.)

Cost: Most workshops are free. Some workshop sites may charge a small fee. CareOregon will reimburse the cost of the workshop when you attend at least four of the six workshop sessions. To be reimbursed:

  • Complete the  Workshop Reimbursement form.
  • Have a workshop leader sign the form.
  • Mail the form to: CareOregon, 315 SW Fifth Ave., Suite 900, Portland, OR 97204, Attn: Health Education.
